Perhaps you should also make some inquiries at Immigration, regarding your status in the USA.

The medical system in Canada
is different than in US and unfortunately the rules are not that well defined. My advice would be to check with Royal college regarding the matter and also CPSO if you plan to do the sub specialty training in Ontario. I think there is a program called repatriation program for physicians trained in US for licensure in Canada
. This is because the requirement for practice license is different in US and Canada
. The residency training is one year longer in Canada
. After this year of training (sometimes they just waive this requirement) you may be eligible for the fellowship as a regular applicant. Or else you may apply for the individual fellowships advertised through the different universities. The best in this case would be to email the program director.

Sorry I did not read your question properly- your main concern is with immigration- you may apply right away. or you may wait till you get a job offer and then apply. Its upto you. For this you should check with immigrations Canada
. After you become a PR it takes 3 years to be a citizen
Hope this helps

ameer,
hi,
Thanks for answering my question. After 3 years residency in medicine
in usa i can go to subspeciality program. the only problem is immigration. as it takes time. but i think if i apply for immigration now then if i get any job i can make my process of immigration fast. yes, there are some states in Canada
who accept 3 years of residency and let do fellowship after that.
thanks again for answering my question.
