Quick Scroll Obstetrics - drug of choice - Pre-eclampsia 07.14.08 (1 month ago) #2

Quote:

Danforth's Obstetrics and Gynecology , 10th Edition 16 - Hypertensive Disorders of Pregnancy>Long-term treatment of hypertension
Drug Starting dosage Comments
Methyldopa 250 mg b.i.d. Rarely indicated
Labetalol100 mg b.i.d. First choice
Atenolol 50 mg q.d. Associated with IUGR
Propanolol 40 mg b.i.d. To be used with associated thyroid disease
Hydralazine 10 mg t.i.d. To be used in cases of left ventricular hypertrophy
Nifedipine 10 mg b.i.d. To be used in women with diabetes
Thiazide diuretic 12.5 mg b.i.d. 50 mg/d Use in salt-sensitive hypertension and/or CHF; may be added as second agent; not to be used if preeclampsia develops or IUGR is present

Williams Obstetrics 22nd edition - Section VII. Obstetrical Complications > Chapter 34. Hypertensive Disorders in Pregnancy
Does not mention Alpha Methyl Dopa,
Generally condemns antihypertensive therapy for pre-eclampsia and
Recommends it only in hypertensive emergencies with diastolic BP>110mm
Also they recommend intravenous labetalol or hydralazine and seem to be wary of using sublingual nifedipine

(However even though text books may not say so, in the Indian context the drug of choice is Alpha Methyl Dopa since Labetolol is not available, Hydralazine and Nifedipine (sub-lingual) can cause precipitate fall in BP. Alpha Dopa is cheap, readily available and easily used)
Hence mark the answer as Labetolol but use Methyl dopa!!
