A 21 years woman is seen in the ED with a history of acetaminophen overdose 4 houres previously. She is drowsy with a respiratory rate of 24/min. Treatment might include all the following EXCEPT.
1-I.V fluids.
2-Enteric charcoal.
3-Naloxon.
4-Intubation.
5-Acetylcystine therapy.

What Naloxone can offer to a patient with acetaminophen overdose ?
acetaminophen is not an opiod.

And in this case the patient is drowsy with a respiratory rate of 24/min , intubation might be needed.

I would say charcoal is wrong...

It is usually only helpful in the first 60min and might be contraindicated in drowsy patient.

Altered mental status might also lead to intubation if airway can't be protected or breathing decreases.

Regarding Naloxon: Patient is drowsy 4h post ingestion, normally in the first 24h there is only N/V, malaise, no mental status change with Acetaminophen. As coningestion is commen, giving Naloxon (a universal antidote with no possible negative effects) might be a good idea.

I.V. Fluids and NAC are obviously right.

George it sure is MCC question and the answer over-there is IV fluids..
i think its bcuz I/V fluids is not the treatment of overdose its just the supportive strategy
