a pt recently underwent a renal allograft transplantation and on trt wth azathioprine n prednisolone he developd fever , cough wth thick sputum and lt. lower lung consolidation wth a cavity. culture specimen demonstrates gram +ve org. wth beaded string appearance. initial trt wll include?

penicillin

erythromycin

ceftazidime

sulfisoxazole

no the ans given is sulfisoxazole.it seems the organism wll b nocardia,but i'm not sure.

anybody wth other views?

the patient has undergone renal transplantation if u refer pg 863 of harrison and pg 1006 u will be able to make out that organism is nocardia becoz gram + with prominent thick cough cavities in lungand moreover in an immunosuppresive patient and so treatment sulfisoxazole reply me if u differ and dont u think s.pneumonia has lanceolate appearance
