A two year old boy is brought to the Accident and Emergency Department by his mother. He has been unable to bear weight since a fall from a chair last night. He is tender over the mid-tibia but there is no obvious limb deformity.

What is the SINGLE most likely radiographic appearance of the fracture?

A Angulated fracture
B Compound fracture
C Epiphyseal fracture
D No fracture likely
E Spiral fracture

Hi everyone,

Lets take the answers point by point.

A. Angulated fracture: not possible as there is no obvious deformity. Tibia is subcutaneous bone and deformity however slight willbe obvious.
B.Compound fracture: Radiographic evidence... very little if any. And with the low energy fall, this possibility is less.
C. Epiphyseal fracture: Epiphyses are located at the ends of the bone not mid-tibia. So this is also ruled out.
D. No fracture likely: Remember we are looking for radiographic evidence, and this is unlikely because with just contusion the boy should be able to bear weight on his limb if not walk.
E.Spiral fracture: Due to the above reasons. No deformity is usually seen. Only swelling and tenderness and inability to bear weight should be the clinical presentation.

So the answer is SPIRAL FRACTURE.
